What is the pelvic floor?

Our pelvic floor is a network of muscles, fascia and ligaments that gives our body support, strength and upright posture. It is responsible for holding the bladder, uterus and bowel in place and for maintaining our continence. Strains such as pregnancy, childbirth or intense sport  can weaken the pelvic floor and impair its functions.

When the pelvic floor no longer performs its tasks properly, this can lead to various problems. The most common symptoms of pelvic floor weakness include:

  • Incontinence (urine leakage or uncontrolled urination)

  • Prolapse of the uterus, bowel or bladder

  • Back pain or herniated disc

  • Pain during sexual intercourse or during menstruation

  • Postural problems and pain in the skeletal system

  • A feeling of pressure and pain in the vaginal area

Why do I do all this?

In order to create the right support and the right training for you, I need to know exactly which problems your pelvic floor is causing you and where its weak points are. Only then can I discuss with you how to proceed and help you in a targeted way.

Check-Up

What does a check-up involve and what are the benefits for me?

The pelvic floor check-up is a multi-step process.

Before our appointment, you will receive a medical history form to fill out.

At the beginning of the appointment, I will have a detailed conversation with you to get a first impression of your pelvic floor problems, possible causes and any habits that may be maintaining them.

After that, I perform a palpation examination, feeling the pelvic floor both externally and vaginally. This involves assessing strength & function, possible signs of prolapse or muscle injury, and whether you are activating your pelvic floor correctly.

This is followed by an ultrasound of your pelvic floor to assess the position of your organs more precisely, to check whether the muscles are intact and how full the bladder is.

Supporting your pelvic floor usually includes a tailored training plan , an adjustment of your daily routine and movement habits , and the fitting of a pessary as a basis for a healthy pelvic floor.

Together we will develop a training concept that will support you step by step in strengthening your pelvic floor and regaining your quality of life.

Pessary

A pessary is a medical device that is often used for incontinence or pelvic organ prolapse. It is a small, flexible silicone device that is inserted into the vagina to support the organs in the pelvis and thus relieve the pelvic floor, comparable to a brace for the knee. The pessary helps to alleviate symptoms such as bladder or uterine prolapse and to prevent urine leakage by supporting the pelvic floor and bringing the organs back into their original position.​

The pessary is a bandage for the pelvic floor.

When can I use a pessary?

After childbirth, the pelvic floor initially lacks the full support it had before. A pessary is not only used if you have pelvic organ prolapse, incontinence , or similar issues, but it also makes sense to have one fitted prophylactically after giving birth . It can help reduce pressure in the pelvis and alleviate discomfort until the pelvic floor regains sufficient strength. Studies show that wearing a pessary leads to a significant improvement in symptoms and accelerates the recovery of the pelvic floor, thus preventing long-term consequences . Once you resume exercising , a pessary can protect your pelvic floor from pressure. It is advisable to wear a pessary during exercise for at least the first two years after giving birth, and even for life if symptoms persist.

What does a
pessary fitting involve?

After a thorough medical history and physical examination, I will work with you to select the appropriate pessary model – tailored to your symptoms and physical requirements. Then I will determine the size and try it on until we find the pessary that fits perfectly and provides you with the desired support and comfort.

The fitting usually takes place in one session. I will give you detailed instructions on how to use, care for , and insert and remove the pessary yourself.

Different models are used depending on the symptoms:

  • After childbirth, a cube pessary or a bar pessary is often used to stabilize the pelvic floor.

  • For stress incontinence (loss of urine during physical exertion), a urethral pessary or a tampon pessary can help.

  • For organ prolapse – for example, of the uterus or bladder – cube, piston or ring pessaries are suitable.

Choosing the right pessary is always individual and depends on your personal needs. Once we've found the right pessary for you, your gynecologist can prescribe it, or you can buy it yourself.
